New Patients

We are happy to register new NHS patients within our practice boundary area.

How to Register

Joining the practice is easy. All eligible people living in the practice’s catchment area can register with Tudor Way Surgery as a regular patient.

If you do not have an address of your own, please state upon the new patient form that you do not have a fixed address. You do not need to provide proof of your address, photographic ID or immigration status to register with us. After you have completed and submitted the form your registration will be processed.

Child Registration

These are the initial registration forms that must all be completed prior to your registration being completed.

If you wish to have access to your medical records online, you must attend the surgery with two forms of identification.

Acceptable identification includes:

  • Passport
  • Birth Certificate
  • HC2 Certificate
  • Rough sleepers’ identity badge
  • Hostel Registration/mail forwarding letter.

Before attempting to register with the practice, please ensure you live within our service boundary by entering your postcode below.

Temporary Residents

If you are not registered with the practice but need treatment from a surgery, you can request to register as a temporary patient.

You can request a Temporary Resident Form from your surgery, you can register as a temporary patient if you are in the area for longer than 24 hours but less than three months.

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.
